Why the Basics Still Matter More Than the Drips

Longevity has become visually spectacular. Marble clinics, IV lounges, hyperbaric chambers and peptide menus promise “advanced optimisation” for those who can afford it.

But beneath the polish sits an uncomfortable truth: many people are being offered experimental tools before their foundations are in place.


At Wellvia, this became clear while designing a longevity programme. When our medical advisory board reviewed a list of frontier interventions, the same question kept surfacing:

“For whom—and before what?”



The Problem with Skipping Ahead


Today’s longevity client is informed, tracked, and measured. They know their HRV, sleep scores, and biomarkers. What they often lack is a joined-up strategy focused on the outcomes that matter most: strength, blood pressure, metabolic control, aerobic capacity, and sleep consistency.

The industry’s implicit promise is seductive: skip the basics, go straight to the cutting edge. Science tells a different story.



The Three-Floor Longevity Stack


Responsible longevity care is sequential:

Floor 1 — Foundations Strength, fitness, metabolic health, sleep, and relationships.

Floor 2 — Medical Prevention Evidence-based screening, risk management, and targeted treatment.

Floor 3 — Frontier Tools IV drips, NAD⁺, hyperbaric oxygen, peptides, plasma exchange.

When clinics lead with Floor 3, they are selling theatre, not longevity.



The Evidence Gap


Most frontier tools are supported by mechanistic or biomarker data, not long-term human outcomes. That doesn’t make them useless—but it does make them experimental.

The ethical failure is not innovation. It is selling outcome-level certainty where none exists.



Why the “Boring” Wins


Across decades of human data, the strongest drivers of health span remain unglamorous:

  • Muscle and strength

  • Cardiorespiratory fitness

  • Blood pressure and glucose control

  • Sleep and circadian rhythm

  • Social connection


These don’t scale like devices, but they compound like nothing else.
